The Washington State Medical Quality Assurance Commission  licenses and regulates physicians and physician assistants. It makes the following information publicly available.

License Verification.
You can find out if a physician is licensed by calling the Commission at 360-236-4800.

Malpractice Information.
The Commission will disclose information on closed malpractice lawsuits brought against physicians and physician assistants. A summary report giving the amount of any payment made and a brief description of the injury is available without charge. The full file, except for the patient's name and medical record, is also available. However, there is a per page charge if the the file is over 50 pages. All malpractice information must be requested in writing. Information about requests can be obtained by calling the Commission at 360-236-4815.


Disciplinary Actions of the Medical Quality Assurance Commission.
Information on closed disciplinary proceedings against physicians and physician assistants is also available. A copy of the complete file of a proceeding, except for the name of the complainant and the patient's medical record, can be obtained by making a written request to the Commission. Information from disciplinary files must be requested in writing. Information about requests can be obtained by calling  the Commission at 360-236-4815.

If a disciplinary proceeding resulted in a finding that there was no professional misconduct, information on the proceeding will only be available to the public for 7 years. There is no time limit on the public availability of information about a disciplinary proceeding which resulted in a finding of professional misconduct.

Information on the disciplinary status of any licensed, registered or certified health care provider, including the number of open and closed complaints, can also be obtained over the telephone by calling 360-664-4111. This is an automated system and can only be used if you have the provider's license number.


Hospital Disciplinary Actions.
Information on disciplinary actions taken by hospitals against physicians and physician assistants is also available. The patient's name will not be disclosed. A request for this information must be made in writing. Information about requests can be obtained by calling  the Commission at 360-236-4815.
